  • Continuum Hospice Care (CHC) is a local not for profit hospice that has for more than 20 years combined unparalleled specialist comfort care with dignity and respect to improve the lives of tens of thousands of New Yorkers.
  • Enabling people to live where they are -- most wish to be at home -- we bring a full array of medical services and necessary supplies.  We visit regularly to make sure we are giving all the nursing care that's needed.  Our team also has social workers and chaplains, if requested.  Volunteers help with tasks they are qualified to do, like massagge, and things that need to be be done, like shopping.  When needed, we help provide home hospice aides.
  • Our physicians are board certified specialists in hospice and palliative care (including pain management).  Dr. Russell Portenoy, one of the nation's reknown experts in controlling pain, is the chief medical officer of our hospice.
  • Our phones are answered by people, not machines. Our on-call staff is on duty 24/7. Hospice physicians, nurses, chaplains and social workers are always available to discuss your hospice needs.
