CHOOSING A HOSPICE

There are many factors to be considered when selecting the right hospice for your needs. To learn more about selection criteria that might be important to you, we recommend you visit the National Hospice and Palliative Care Organization's website at www.nhpco.org .

Continuum Hospice Care is the right choice for most patients and families for many reasons. The scope and quality of our services are unsurpassed in New York City.   • Continuum Hospice Care/Jacob Perlow Hospice is one of the few hospices in the United States that practices full Open Access, meaning our patients may continue helpful palliative treatments while in our care.
  • We are committed to speed of access. Our admission specialists are available to answer questions and complete admissions 7 days a week. We are available when you are.
  • Our medical staff has specialist level training in palliative care and pain management. In fact, all hospice employees and volunteers must go through an extensive hospice training program.
