Continuum Hospice Care/Jacob Perlow Hospice is one of the few hospices in the United States that practices full Open Access, meaning our patients may continue helpful palliative treatments while in our care.
- We are committed to speed of access. Our admission specialists are available to answer questions and complete admissions 7 days a week. We are available when you are.
- Our medical staff has specialist level training in palliative care and pain management. In fact, all hospice employees and volunteers must go through an extensive hospice training program.
