Make a Contribution

Your charitable gift will help us bring comfort and peace to people at life's end and console their heartbroken families. Philanthropic funds are used for expansion so we may care for an increasing number of New Yorkers in need of hospice care, and for special programs to enhance the quality of life of our patients in their final days.

There are many ways you can make a contribution to Continuum Hospice Care:

Memorial Gifts – Memorial gifts are a wonderful way to remember or honor a loved one while touching another person's life. Often, families ask that, in lieu of flowers, memorial gifts be made to Continuum Hospice Care.

Champions for Hospice – An unrestricted annual gift of $1,000 or more will admit you to Champions for Hospice, an exclusive group of hospice's most steadfast supporters. Our Champions receive invitations to special receptions and events, news about the latest medical or legislative events that affect hospice care, and other interesting opportunities

Tree of Life – The Tree of Life is a permanent, hand-crafted cast bronze sculpture displayed in the Jacob Perlow Hospice Inpatient Unit, located in Beth Israel Medical Center in Manhattan . A gift of $1,000 will be recognized with a bronze-colored leaf on the Tree. Gifts of $3,000 will receive a gold-colored leaf, and gifts of $10,000 will be recognized with stones placed at the base of the Tree.

Bequests and Planned Giving – There are many ways you can make a legacy gift to Continuum Hospice Care:

  • Name Continuum Hospice Care as a beneficiary in your will, living trust or life insurance policy
  • Arrange a charitable remainder trust that meets your needs for lifetime income and can provide significant income tax and capital gains tax benefits
  • Make a gift of real estate, appreciated stock or securities

These gifts often provide a substantial tax advantage. We will be happy to work with your financial advisor or attorney to discuss the will or estate gift you are considering, or to facilitate the transfer of a stock gift.

Volunteer

An essential part of the hospice community, volunteers are compassionate people who generously give their time, goodwill, and unique talents to bring love and comfort to our patients and their families. There are so many ways to help:

  • Companionship
  • Support for a patient who is alone
  • Respite for a caregiver
  • Taking a patient for a walk
  • Reading to a patient
  • Helping out around the house
  • Babysitting
  • Helping to write a letter
  • Assisting with meals
  • Visiting with family
  • Sharing artistic and professional skills
  • Clerical / administrative support in the Hospice offices

Our volunteers frequently tell us that they find the time spent with our patients and their families to be richly rewarding. We hope you will join our group of exceptional volunteers and experience the warmth and love of our Hospice community.

Share your hospice story

Unfortunately, many people don't understand hospice. They don't realize that hospice can help people who still have months to live, and that hospice care can be received in a person's own home. They aren't aware that hospice is paid for by most private insurance or Medicare. Most of all, they don't know that hospice is about living to the fullest for as long as one can.
