OUR FACILITIES

While about 95% of our patients are able to stay in the familiar surroundings of their homes, Continuum Hospice Care provides premium care for patients with more acute care needs. Patients may be transferred from home or a nursing home to an inpatient unit for acute care, or when the primary caregiver needs a longer respite. When the crisis has passed, the patient may return home. Additionally, for patients who do not require inpatient admittance, but for any reason are unable to stay in their homes, we offer a hospice residence. Continuum Hospice Care facilities include:

The Mollie and Jack Zicklin Jewish Hospice Residence

Additionally, we are the hospice provider of choice for more than 25 nursing homes and hospitals located throughout New York City.
