HOSPICE CARE AT HOME

Most of our patients dream of being in their own homes throughout their final days. Our goal is to help them experience the last stage of life in the manner they wish. Continuum Hospice Care provides the medical care and support that can make this basic human entitlement a reality.

Our services include regularly scheduled home visits by specially trained Hospice Team members to evaluate the patient's condition, supervise care, assure comfort, meet specific needs and resolve problems. Through an interdisciplinary care team approach, the Hospice Team educates and instructs family members or primary caregivers on how to manage physical and emotional symptoms such as pain, anxiety or confusion.

Home hospice services also include:

  • Medical expertise of specially trained hospice physicians to establish and carry out a unique and comprehensive Plan of Care that treats each of our patients for pain and other symptoms. Also, the patient's personal physician may consult with our physicians on the patient's disease and, when appropriate, other specialists may be involved.
  • A nurse case manager who:
    • Regularly monitors and attends to the patient's condition
    • Is the primary contact
    • Orders and coordinates all medical matters including medications, tests, complementary therapies, equipment, materials, home health aides, supplies, etc.
  • A social worker to:
    • Counsel and encourage coping
    • Make financial assessments
    • Make referrals for food stamps, housing, or other services
    • Help the family with final arrangements, when necessary
  • Specially trained chaplains of all faiths for consultation, conversation and to help identify and address spiritual concerns
  • Bereavement services to help loved ones grieve appropriately and heal
