Our Leadership

Through our position as New York City's premier hospice, our mission is to
increase hospice access and serve all eligible patients who want our care.

We believe that every person should have the opportunity to prepare for the end of life with support that recognizes his or her physical, emotional, social and spiritual needs. Hospice provides compassionate care for the patient and family so that they may live as fully and comfortably as possible. Continuum Hospice Care/Jacob Perlow Hospice is a caring community committed to providing such support through its interdisciplinary services and skillful symptom management.

We are dedicated to providing quality care to people living with advanced illness, and committed to enhancing community awareness and knowledge of end-of-life issues. Our intention is that health care systems will use hospice and employ palliative care, ultimately promoting a more humane society through increased attention to the needs of each individual facing the end of life.

Every year we serve more than 2,500 families. Ninety-five percent of our patients receive hospice care at home to allow peace, comfort and dignity during their final days. Nearly three-quarters are 65 or older, and over 80% of our funding comes from Medicare and Medicaid.

One of New York's largest hospices, Continuum Hospice Care/Jacob Perlow Hospice is a national model for Open Access Hospice. In just 4 years, under the direction of Carolyn Cassin, President and CEO, Continuum Hospice Care has more than quadrupled the number of people we care for, to almost 400 patients and their families, each day. Our overnight facilities include the Jacob Perlow Hospice inpatient unit at Beth Israel and the Mollie and Jack Zicklin Jewish Hospice Residence in Riverdale. A member of Continuum Health Partners, Inc., Continuum Hospice Care employs 225 full-time staff and we have 230 specially trained volunteers.
